This is what We found....Even after the Factory reset his phone would only dial from a headset (2nd button up)

I changed it to "None" and it now works fine, strange that the Factory Reset did not set it back to "None".
The interesting thing while I was looking for the issue I found out about the setting from Headset to Speaker as a default without logging into the phone from the PC
I guess by choosing from the PC you can hard code it to either Speaker, Headset, Bluetooth or None.
HEADSET MODE: Headset or Bluetooth
1. Press "Headset Button" and dial an extension or press a preprogrammed button will leave it in the Headset mode.
Note: it will stay in that mode until you change it.
SPEAKER MODE
2. Press "Speaker Button" and dial an extension or press a preprogrammed button will leave it in the Speaker mode.
Note: it will stay in that mode until you change it.
BLUETOOTH SETUP:
Example: Bluetooth setup for my JBL T450BT Headset
1. Put device in pairing mode
2. Scan for Bluetooth devices and locate the device you want and then press Connect.
3. Go to Pin and set the Pairing code eg. 0000 (00 wait 00 wait 00 wait 00)
4. Press Connect, when connected it will have a red check mark on the selected device
Using Bluetooth when connected to your device and on a call:
1. Press "Headset Button" and dial an extension or press a preprogrammed button.
2. To switch between back and forth press the Headset or Speaker while in a call.
I changed it to "None" and it now works fine, strange that the Factory Reset did not set it back to "None".
